In order to receive more detailed information pertaining to investments offered by PROFIMEX, I hereby affirm with my signature below that I declare and agree to the following:

1. I, as an individual investor, fulfill one of the requirements listed below (please check the appropriate box):

   I own liquid assets-cash, deposits, financial assets, and securities-worth in excess of NIS 8 million (as adjusted according to Israel's Consumer Price Index)I have an annual income of at least NIS 1.2 million (as adjusted according to Israel's Consumer Price Index) in each of the last two years, or, and annual combined household income of at least NIS 1.8 million (as adjusted according to Israel's Consumer Price Index) in each of the last two yearsI own liquid assets-cash, deposits, financial assets, and securities-worth in excess of NIS 5 million (as adjusted according to Israel's Consumer Price Index) and an annual income of at least NIS 600 thousand (as adjusted according to Israel's Consumer Price Index) in each of the last two years, or an annual combined household income of at least NIS 900 thousand (as adjusted according to Israel's Consumer Price Index) in each of the last two years

   I represent a corporation that fulfills one of the requirements listed below:

   The corporation is under the complete ownership of an individual(s) that fulfill(s) section 1 aboveThe corporation’s equity is valued at more than 50 million NIS and was not incorporated in order to purchase securities

2. Additionally, I agree to receive further detailed information regarding possible investments.

3. In the event that my financial status, as attested to in Section 1 above changes, I will immediately inform PROFIMEX about these changes.

4. I hereby agree that all information shared in this declaration shall be held, managed, and used by PROFIMEX in proposing investment opportunities.

5. I am aware that PROFIMEX relies on the statements contained here in order to propose detailed and specific investment opportunities and that statements on my part, that misrepresent or are otherwise inaccurate, incomplete or imprecise, may cause damages to PROFIMEX.

I am aware that upon joining an investment, I will have to provide the appropriate documentation to support
this declaration.

INVESTOR DETAILS

1. Liquid assets - This refers to cash, deposits, financial assets and securities as defined by the Regulation of Investment Advice, Investment Marketing and Investment Portfolio Management Law of 1995. Financial Assets - Instruments as defined by the Joint Investment Trust Law of 1994, stocks or shares in Funds registered outside of Israel, options, futures, structured or indexed products and pension funds.

2. Family Unit - Defined as an individual and his/her household living with him/her and/or dependents.
